What will happen to the disabled when Social Security and Medicare ends?

There has been a lot of talk lately about ending Social Security and Medicare. But what is the plan for those who are disabled? Some people have been ill and disabled for a long time and were not able to save for their future or prepare for the demise of these government entitlement programs. So, what'll happen to them? Does the government have any ideas on how to care for them or are they headed for life on the streets?
